All employees 和 students planning to travel to foreign destinations on 基督教社会联盟 business must request approval from our campus President prior to their departure date. 旅行 to countries on the US Department of State, Bureau of Consular Affairs Current 旅行 Warning List or those deemed high hazard risk (also known as "War Risk" countries) requires approval from the Chancellor’s Office. Prior to making any travel arrangements you should contact the Office of International Education for guidance.

• 完成 Foundation 旅行 Request Form [PDF] if your travel is h和led through the Foundation.
• 完成 基督教社会联盟DH International 旅行 Authorization Form [PDF]
• 完成 基督教社会联盟DH Participant list [PDF] if you are hosting a faculty-led study abroad program with students.
• Students must complete the Student 旅行 Waiver form [PDF], If travel is h和led through Foundation.
• Students must complete the Release of Liability Form [PDF], if travel is funded via state side.
• 完成 Certification Acknowledgement Form

California State University Risk Management Authority (基督教社会联盟RMA) Foreign 旅行 Insurance Program (FTIP)

所有教师, staff 和 students who travel internationally on 基督教社会联盟 business are required to use California State University Risk Management Authority (基督教社会联盟RMA) Foreign 旅行 Insurance Program (FTIP) 和 report their trips as soon as possible, but no less than 30 days prior to the departure date for high hazard areas. The Office of International Education will work with Risk Management to obtain the foreign travel insurance once international travel has been approved by the President.

FTIP benefits include, but are not limited to, the following:

• General Liability 和 Excess Auto Liability
• Primary Medical Expense, Emergency Medical Benefits 和 Evacuation
• Repatriation of Remains
• Political Evacuations 和 Repatriation Benefit War Risk Coverage
• Accidental Death 和 Dismemberment Benefit

It is recommended that all faculty, students 和 staff traveling internationally sign up for the State Department’s Safe 旅行er Enrollment Program (STEP).

The STEP provides information to the traveler should conditions change in the foreign location 和 can assist in an emergency. The traveler need only enroll once 和 then can update as trips are planned.

美国.S. Department of State has a Smart 旅行er App available in the iTunes商店 和 Google Play Store. The app is the official State Department app for U.S. travelers 和 provides easy access to frequently updated official country information, 旅行警报, travel warnings, 地图, U.S. embassy locations, 和 more.
